Click on the "Submit A New Request" button in the top right of the Elite Help Center to access the form list.
Request Forms
On the request forms page, you will see the following forms:
- General Support Request: Report questions, issues, or errors to the Elite Customer Support team.
- Template Report/Dashboard Request: Request a new report, template, or dashboard in Elite, or request changes to an existing one. Please complete every field so we can assist you faster than ever!
At the top of each form, you will be asked to enter your email address. Please use your company email address to ensure your ticket gets logged correctly and reduce delays.
